INSTRUCTIONS FOR PARENTS:

Adult name & Birthday will be entered first followed by the child's information.

IMPORTANT!
1) Waivers for minors must be signed by a PARENT (or COURT APPOINTED legal guardian).
2) Waivers signed by a friend, relative or group leader WILL NOT BE ACCEPTED.
